Pre and Post Operative Instructions

At Dr. Barrett Plastic Surgery, PC (“BPS”), we are committed to offering our state-of-the-art plastic surgery services to a limited number of eligible patients in need who cannot afford the cost of a procedure we offer. The Barrett Gives Back program applies only to procedures we regularly perform that are provided at our facility in Beverly Hills. We do not cover the cost of travel or lodging in connection with a procedure.

You may be eligible if:

  • You are a US citizen or permanent resident;
  • You are over the age of 13;
  • You are without insurance coverage, Medicaid, Medicare, or any other government or other healthcare coverage for the procedure you seek;
  • Your income and monetary assets are such that you are unable to afford the procedure without financial assistance; and
  • The procedure you seek is one that we regularly perform.

We may ask for documentation supporting your application, which will remain confidential subject to our privacy policy (see below) and the Health Insurance Portability and Accountability Act (“HIPAA”).  

The criterion for selecting Barrett Gives Back patients is completely within our sole and absolute discretion. We don’t guarantee that any prospective applicant, regardless of need or type of procedure, will get priority over any other prospective applicant we may select. However, eligibility for our Barrett Gives Back program is determined without regard to sex, race, color, religion, ancestry, national origin, age, disability, medical condition, marital status, sexual orientation, gender identity, gender expression or educational background.

If you are selected to be a Barrett Gives Back patient you hereby acknowledge and agree that, in consideration for the medical services and/or advice provided to you, you grant to BPS and its designee(s) a non-exclusive, irrevocable, sublicensable, perpetual, worldwide right and license to use, publish, distribute and exhibit your name, image, likeness, voice, and biography (“Your Personal Information”) by any and all means and media, whether now known or later devised, for marketing, promotional, advertising, informational and/or educational purposes, including without limitation on BPS’ websites, social media pages and channels and in BPS’ marketing, promotional and advertising materials.  Notwithstanding the foregoing, your Personally Identifiable Information (as defined below) and your Protected Health Information (under HIPAA) shall not be used for marketing or promotional purposes in consideration for medical services and/or advice and your decision to allow us to use such private information in marketing or other materials shall be your own decision and shall not be a condition of your obtaining medical services and/or advice hereunder. Further, you hereby acknowledge and agree that neither BPS nor any of its designees has any obligation whatsoever to publicize Your Personal Information.

You may submit your application on our website, located at https://www.drdanielbarrett.com/barrett-gives-back (“Our Website”). The review and acceptance process may take 30 days or longer from our receipt of your application.  

Privacy Policy

BPS owns and operates Our Website. This BPS Privacy Policy (“Privacy Policy”) describes how BPS collects information through Our Website. The BPS Privacy Policy explains why BPS collects this information, how it will be used or disclosed, and how a visitor may update or delete any identifiable information from our records.

By accessing, browsing, and/or using Our Website, a visitor is deemed to have accepted the terms of the BPS Privacy Policy. Please do not access Our Website if you do not agree or are unwilling to accept the terms of the BPS Privacy Policy.

The BPS Privacy Policy is inapplicable to BPS’ offline information collection, policy, or practices, which are subject to HIPAA Patient Rights and Privacy Policy.

BPS gathers information which can generally be categorized as either “Anonymous Information” or “Personally Identifiable Information.”

Anonymous information (occasionally referred to as “Aggregate Information”) is information which cannot be traced back to any specific individual. BPS uses this Anonymous Information to understand how visitors use Our Website and how to improve Our Website based on this Anonymous Information. BPS may collect the visitor’s domain name, referral data (the address of the last URL a user visited prior to clicking through to Our Website), the browser used to access Our Website (Google Chrome, Safari, Firefox, Microsoft Edge, etc.), and platform type (Windows, Mac, Linux, etc.). In addition to the above, BPS will collect aggregate queries for internal reporting. BPS will count, track, and aggregate the visitor’s activity into our general traffic flow analysis to determine where traffic to Our Website comes from, how traffic flows within the BPS site, etc.)

“Personally Identifiable Information” is information provided by a visitor to Our Website that allows BPS to learn specific facts about you for the selection process. The Personally Identifiable Information provided could range from a visitor’s name, address, and zip code to specific health information provided to BPS on an admission form or otherwise. Your Personally Identifiable Information collected on Our Website will only be used to allow the processing of your application and to perform services. If a visitor completes an admission form downloaded from Our Website and submits the form to BPS, BPS uses the Personally Identifiable Information only for the purpose for which it was submitted to BPS. This may include BPS providing this information to third parties with whom BPS has a relationship with to facilitate the delivery of the information or services required.

As part of this Privacy Policy, BPS expressly reserves the right to disclose any visitor information that is already publicly available or that is posted by you (or by others with your permission) to a chat room, forum, bulletin board, social media account, or any other public forum. BPS may also use the Personally Identifiable Information provided to send you information BPS believes may be of interest to you (such as newsletters or other items related to BPS and its medical community) based on information previously provided to us. You have the right to opt out of this information, described below in “Your Opt Out Right.” You can visit the BPS site without providing any Personally Identifiable Information but if you wish to obtain BPS services, certain personal information must be provided.

Security

BPS takes precautions to ensure the confidentiality of your Personally Identifiable Information. However, you must be aware that as you navigate Our Website that any message sent through Our Website or email is not secure and is susceptible to third party interception. Due to the security risk, any Personally Identifiable Information transmitted to BPS is done so at your own risk. BPS cannot be held liable for any Personally Identifiable Information intercepted by any third party.

Cookies

BPS may place “cookies,” small text files used to automatically identify your browser, store, and retrieve login information on a user’s system, in your browser’s file storage area on your computer’s hard drive. Cookies are small text files stored on a site visitor’s computer to gather and track information related to you. Cookies store your preferences through a password you select to access a website. Cookies help website administrators review patterns and improve websites. While you may have the ability to block cookies through your browser’s settings, this may prevent usage of Our Website and successfully applying to the Barrett Gives Back program. Any information collected via cookies is not linked to any Personally Identifiable Information provided to BPS.

Log Files

BPS collects and logs IP addresses to analyze website visitation trends and to help administer Our Website. An IP address is a number automatically assigned to your computer whenever the internet is accessed which allow computers and servers to recognize and communicate with one another. BPS uses IP address information to administer Our Website, gather aggregate information on visitors, diagnose problems with the BPS servers, and better understand how Our Website is being used. BPS may share this aggregate information with third parties, such as our staff, suppliers, and potentially other businesses. BPS does not associate IP addresses with records containing Personally Identifiable Information to maintain visitor anonymity. As such, IP addresses of visitors are not linked to any Personally Identifiable Information.

Your Opt-Out Right

As indicated in this Privacy Policy, BPS may use any information provided to send you information and materials BPS think may be of interest to you. Should you not wish to receive any information or materials from BPS, including emails or newsletters, you have the right to opt out, at any time, of such disclosure and additional use of your information. To opt out, you must check the provided appropriate box at the time you are requested to provide your Personally Identifiable Information. Your preference can be changed at any time by contacting BPS at the address provided.

Our Website contains links to third party websites that are not operated, or in any way affiliated, with BPS. Should you click on one of these links you may be asked or required by these third-party websites to provide or release Personally Identifiable Information that you may have provided on Our Website. You are never required to release this information and the choice is entirely up to you. Refusing to provide this information in no way effects your eligibility for the Barrett Gives Back program or the validity of your application.

Should you choose to provide any Personally Identifiable Information to these third-party websites, please be aware that BPS is not responsible for, and takes no responsibility for, the privacy practices of these third party websites, nor any adverse consequences which may result from interaction with these third-party websites.

BPS has no control or influence over third-party website privacy practices, use of cookies, or other electronically collected data. BPS encourages caution when leaving Our Website and review of the privacy policies of all websites which collect Personally Identifiable Information. The BPS privacy policy does not apply to any information collected through third-party websites. BPS cannot be held liable for any Personally Identifiable Information intercepted on third-party websites.
