April had a history of multiple breast revision surgeries, beginning with her first augmentation at age 18. She underwent a revision at 21, and had her implants removed and replaced again in 2022, all performed by the same surgeon. She previously had 450cc moderate profile silicone implants placed under the muscle via the transaxillary approach. She presented with asymmetry, with the right breast sitting lower than the left. The left breast was hard, painful, and tight to the touch. Both breasts showed signs of a double bubble deformity. She also reported nerve pain in the axillary area, describing it as a shooting or "pins and needles" sensation, similar to when a limb falls asleep. During her last surgery, she underwent axillary liposuction and had a lump removed. Her goals included achieving fuller, more symmetrical breasts, lifting the right breast, relieving pain in the left breast, and improving overall breast shape.
Procedure Details
Dr. Barrett performed an explant of both previous 445cc Allergan implants and removed the capsule in the left breast that gave her grade four capsular contracture. The implants were prepared and soaked in a triple antibiotic solution, then inserted into each breast using a no-touch technique with a Keller funnel. Gloves were changed prior to handling the funnel and implant for each side to maintain sterility. The incisions were meticulously closed in five layers to promote optimal long-term healing and aesthetic results.
