The Pros & Cons of Preservé: Minimal Downtime, Maximal Results

The Natural Plastic Surgeon Blog

Regardless of ever-changing trends, one thing that's remained consistent throughout the history of plastic surgery is the popularity of breast augmentations. What has changed is an increased desire for minimally invasive options and natural results instead of the intensity of old-fashioned implants. Because of this, there are constant innovations in how surgeons perform the procedure, with the latest being the Preservé technique. To address the main patient concerns, these augmentations stand out by maintaining the breast's natural anatomy and function, while providing a safe and comfortable implant or fat transfer option.

For those seeking the most natural and precise results, Preservé is an excellent choice. Barrett Plastic Surgery is excited to offer Preservé as an option for clients seeking quicker recovery, natural results, and increased safety. Dr. Barrett praises the technique's ability to manipulate breast tissue while preserving its organic structure, making it a great choice for subtle yet detailed augmentation. Keep reading as we break down what the technique entails, the pros and cons, and if it's a good choice for you.

What is Preservé?

Historically, breast augmentations involve making incisions on the patient's chest to insert a customized implant suited to the client's desired results. Implants are usually silicone or saline, with silicone implants pre-filled to the patient's preference and saline implants filled after the casing is already placed inside the chest. The problem with either preference is the possibility of implant rupture or movement, as well as damage to the breast tissue from the incisions. It can be an intense surgery with an uncomfortable and long recovery period.

Preservé solves these problems by introducing a new technique for placing and securing the implant and by using Ergonomix materials to increase comfort and reduce the risk of rupture.

The Technique: This method avoids the need for extensive incisions by using a specialized instrument called a Channel Separator to create a tunnel for implant placement without cutting the tissue. The only incision necessary is a small cut in the inframammary fold under the breast to insert the device.

Next, the surgeon inserts an inflatable balloon device inside that tunnel to gradually move the breast tissue aside and create a precise pocket for the implant. The device expands once inside the breast, allowing the initial incision to remain small while still providing the necessary volume. This way, your implant and your natural tissue can coexist comfortably inside your breast without unnecessary scarring.

The Implant: In addition to having a less invasive insertion method, this type of augmentation utilizes Motiva SmoothSilk Ergonomix implants for a more customizable experience. These implants have a unique surface designed to enhance biocompatibility, and they adapt as the body changes positions, allowing for a round shape when lying down and a teardrop shape when standing up, aligning with the natural movement of breast tissue.

How is it different from traditional techniques?

As the name indicates, Preservé is all about preserving as much of the chest's natural anatomy as possible. By maintaining the patient's breast's inner support structure, not only is the implant more stable, but there are fewer complications to worry about. Increased conservation of natural tissue also maintains the chest's function, including nipple sensation and muscle integrity. In addition to structural benefits, the type of implant used during the procedure tends to cause less inflammation than other options and helps keep the surrounding tissue soft and malleable.

Other benefits include:

  • Minimal scarring (the initial incision is only around 3cm)
  • Small incisions are less likely to become infected
  • Enhanced stability reduces the chances of the implant drifting or bottoming out
  • Motiva SmoothSilk technology makes the implants more realistic and less detectable to the naked eye
  • Preservé recipients tend to have higher long-term satisfaction rates as the implants stay soft over time

Is it an invasive procedure?

Since Preservé requires a small incision that doesn't disturb the body's muscles or tissues, the method is significantly less invasive than traditional techniques. The surgeon only needs to use local anesthesia, circumventing the need to go fully under. During the procedure, the surgeon numbs the area before making the incision and inserting the Channel Separator to ensure comfort and relaxation.

The entire procedure is faster than traditional methods — it's sometimes completed within half an hour — leading to shorter downtime and streamlined healing. Instead of going to the hospital and anticipating the side effects of general anesthesia, clients can have the surgery performed in-clinic and return home immediately afterward. Patients tend to experience less soreness and bruising and return to normal activities more quickly, sometimes needing only over-the-counter pain management. Some people jump back into their routine within a week or a few days, depending on their personal preferences.

What are the drawbacks?

Because of the unique method that prioritizes small incisions, implant sizing is more restricted. Those who want dramatic results that jump multiple cup sizes may want to consider another option. Patients who want to avoid implants altogether should consider fat grafting for a subtler increase in volume.

Another drawback to Preservé is that it's intended for first-time augmentation only. It's not a strategy for reconstruction. Patients must also have enough existing breast tissue to undergo the procedure.

Am I a candidate?

Preservé is great for people looking for extremely natural results with minimal scarring. Patients who wish to go up one or two cup sizes and have a relatively calm healing process are great candidates for this procedure. Those with active lifestyles benefit from the shorter downtime as they can return to low-impact exercise within days. It's a good option for those who have anxieties regarding anesthesia, loss of nipple sensation, or long-term complications like capsular contracture. This method can also be combined with a lift for more customization and refinement.

Is Preservé safe?

This procedure is based on meticulous research that maximizes comfort and efficacy. The tools used for the surgery are patented technology designed to reduce complications and recovery pain.

It's very important to ensure your doctor is a board-certified plastic surgeon like Dr. Barrett. Just because the procedure is less invasive and can be performed in the clinic does not mean it's safe to see an unqualified provider.
